Forma (thisisforma.co.uk) collects the information you provide when you register and build your profile: your name, email address, and the text, images, and video content you choose to add.
We use this to run the service, keep your account secure, and display your public profile to visitors. We do not sell your data to anyone.
Profile text and account data is stored in a PostgreSQL database. Media files, including video testimonials, are stored on Amazon S3. Video testimonials are transcribed using OpenAI Whisper and processed by OpenAI GPT-4o to generate pull quotes. Forma operates under a zero data retention agreement with OpenAI. Data sent to OpenAI for transcription and processing is not stored, logged, or used to train their models.
By agreeing to these terms, clients who submit a video testimonial consent to their name, likeness, and review being displayed publicly on the practitioner's Forma profile page.
If you are willing for your testimonial to be used in Forma's own marketing, including on our website and social channels, you can confirm this separately via a checkbox on your profile page. You can withdraw this consent at any time by unchecking that box or contacting us directly.
We set cookies only where they are strictly necessary for sign-in and security. These do not require your consent and cannot be turned off while you use the account.
You can delete your account at any time in Settings. When you do, your data is deleted immediately. This includes your profile, any associated media, and all testimonial content linked to your account.
