By submitting a video review through this link, you agree to the following.
Your video, name, and the details you provide will be displayed publicly on your practitioner's Forma profile page. Anyone visiting that page will be able to see your submission.
You confirm that what you have said is true and reflects your genuine experience.
Forma stores your video securely on Amazon S3. It is transcribed and summarised using OpenAI's tools under a zero data retention agreement, meaning OpenAI does not store or retain your data after processing.
Your practitioner can remove your video from their profile at any time. If you want your submission removed, contact us at tom@thisisforma.co.uk and we will action it within 5 business days.
During submission you will be asked whether you are happy for your video to appear in Forma's own marketing, including our website and social channels. This is optional and separate from the profile display above. You can withdraw this consent at any time by contacting us at the address above.
