Terms and conditions

By using Forma you agree to use the service lawfully and only for its intended purpose: creating and sharing a professional profile for health and wellness practitioners. You are responsible for the truth and accuracy of what you publish, and for having permission to use any photos, logos, or other content you add.

Subscription and billing

Forma is a monthly subscription at £29/month, billed through Stripe. Your first month is free; your card is required upfront. You can cancel at any time in Settings and will not be charged again after your current period ends. We do not offer refunds for partial months.

Video testimonials and client content

Clients who submit a video testimonial via your unique link consent to their name, likeness, and review being displayed publicly on your Forma profile. If a client asks for their testimonial to be removed, you are responsible for removing it promptly via your dashboard.

If a client has opted in to Forma marketing use via the checkbox on their submission, their content may also appear on Forma's own website and social channels. They can withdraw this consent at any time by contacting us directly.

Account security

Keep your password confidential. You are responsible for activity on your account unless you notify us within 48 hours of believing it has been compromised.

Account deletion

You can stop using Forma at any time by deleting your account in Settings. When you do, your data and all associated content is deleted immediately. We may suspend or close accounts that breach these terms.

Changes to the service

We may change or suspend the service, or these terms, where we need to. For material changes we will give you notice in the product.

Liability

The service is provided on an as-is basis to the maximum extent allowed by law. We are not liable for third-party content you link to, or for losses that are not a reasonably foreseeable result of our breach of these terms.

Governing law

These terms are governed by the laws of England and Wales. The courts of England and Wales have exclusive jurisdiction, unless you live elsewhere in the United Kingdom, in which case you may have additional statutory rights.
